Connecticut prescribes this one: a notice of intent to claim a mechanic's lien under Conn. Gen. Stat. § 49-35, which you send before the lien and which the statute says what to put in. Plenty of claims are paid at this stage and never become liens at all. One flat fee of $99 — you serve it, and your instructions say to whom, how, and how long you must wait afterwards.
